The Effect of Reminder Drinking Drugs (PMO) on the Level of Compliance with Type 2 Diabetes Mellitus Patients at the North Sumatra University Hospital in Medan City

Introduction

Diabetes mellitus (DM) is a metabolic disorder characterized by high levels of glucose in the blood, which is caused by a lack of insulin production, lack of insulin action, or a combination of both. Effective management of DM requires a good understanding of this disease, so that patients can comply with the given treatment. Compliance in undergoing treatment is very important to control blood glucose levels and reduce the risk of long-term complications. One of the interventions that can increase patient compliance is the use of a drug reminder (PMO). This study aims to determine the effect of PMO on the level of compliance with type 2 diabetes mellitus patients at the Hospital of the University of North Sumatra, Medan.

Background

Diabetes mellitus is a chronic disease that affects millions of people worldwide. The management of DM requires a multidisciplinary approach, including medication, diet, and lifestyle changes. Compliance with medication is a critical factor in the management of DM, as it helps to control blood glucose levels and reduce the risk of complications. However, non-adherence to medication is a common problem among patients with DM, which can lead to poor health outcomes.

Methodology

This study used the Cohort prospective design, which was carried out from August to November 2018. As many as 30 respondents were involved in this study. The research subjects were given interventions in the form of PMO digital applications installed on their smartphones. To measure knowledge, the Diabetes Knowledge Questionnaire-24 (DKQ-24) questionnaire was used, while the level of compliance was measured by the Morisky Medication Adherence Scale-8 (MMAS-8). Data obtained from respondents were analyzed using Microsoft Excel and Spearman test statistics to determine the correlation between knowledge and compliance.

Results

The results showed that there were no significant differences in the level of knowledge and compliance based on demographic factors such as gender, age, education, occupation, and duration of DM. However, further analysis with the Spearman test shows a significance value (2-tailed) of 0.301 (p≤0.05) which shows a positive correlation between the level of knowledge and compliance with patients, even with a low correlation rate (0.195).

Discussion

The results of this study suggest that the use of drug reminder has the potential to increase knowledge and compliance with patients with type 2 diabetes mellitus. Increased knowledge is expected to encourage patients to be more disciplined in taking drugs, so that it can help control their blood glucose levels and reduce the risk of complications that can arise due to this disease. With the increase in compliance thanks to the use of PMOs, patients are expected to be able to take more regular treatment, so that their quality of life will improve.

Conclusion

In conclusion, the use of drug reminder has the potential to increase knowledge and compliance with patients with type 2 diabetes mellitus. This study suggests that the implementation of digital technology in supporting diabetes mellitus patients to be more obedient to their treatment is important. Therefore, it is recommended that hospitals and healthcare providers consider the implementation of digital technology in supporting diabetes mellitus patients to improve their compliance with treatment.
