The Influence of Organizational and Social Image on the Use of Obstetric and Neonatal Services Basic Emergency at the Hamparan Perak Health Center in Deli Serdang Regency in 2018
Introduction
The use of obstetric and neonatal services basic emergency (PONED) is a crucial initiative aimed at reducing maternal mortality (MMR) in Indonesia. However, in 2017, the use of PONED services at the Hamparan Perak Health Center in Deli Serdang Regency was still very low, with only 12% of the total targets of pregnant women utilizing the service. This study aims to analyze how organizational factors and social images influence the use of PONED in the region.
Research Background
This study employed a cross-sectional design and involved 100 pregnant women from a population of 2634 in thirteen villages in Hamparan Perak District. Data collection was carried out through interviews using questionnaires, aimed at finding out various factors that influence the use of PONED, including the availability of human resources, physical facilities, geographical access, available information, and social images related to reputation and services provided by health workers.
The Importance of PONED Services
PONED services are essential for reducing maternal mortality and improving public health outcomes. The World Health Organization (WHO) recommends that all pregnant women have access to emergency obstetric care, including PONED services. Research Findings
The results of the analysis using double logistics regression show that there is a significant effect of geographical access (OR = 9,969; p <0.001), information (OR = 4,349; p = 0.005), and the quality of service provided (OR = 11.024; p <0.001) against the utilization of PONED. This indicates that pregnant women who have good access both in geography and information, as well as provide a positive assessment of the services received, tend to use more PONED services.
Analysis and Explanation
From the results of the study, it is clear that organizational factors such as the availability of human resources and physical facilities have a major effect on the decision of pregnant women to use PONED. If health facilities have good access and doctors and competent medical personnel, the public will trust more to use this service. In addition, clear and easily accessible information about existing services is very important. This means that marketing and communication efforts from the Puskesmas must be improved to be more informative and easily understood by the community.
Social image or health service reputation also has a great impact on public trust. If the community feels that health services, especially in terms of PONED, have a good and reliable reputation, they are more likely to take advantage of these services. Therefore, there is a need for intervention from the Health Office and other related parties to create and maintain the positive image.
Recommendations
Referring to the findings above, it is recommended that the Health Office and Puskesmas in the Silver stretch seeking to improve access, as well as improve the image and quality of PONED services. Concrete steps such as training for health workers, the provision of adequate facilities, and information campaign programs about the benefits of PONED can help increase the use of this service. Increasing public trust in health services is the key to reducing maternal mortality and achieving better public health goals.

In conclusion, this study highlights the importance of organizational factors and social images in influencing the use of PONED services in Deli Serdang Regency. The findings of this study suggest that improving access, information, and the quality of service provided can increase the use of PONED services. Therefore, it is recommended that the Health Office and Puskesmas in the Silver stretch take concrete steps to improve the use of PONED services, including training for health workers, the provision of adequate facilities, and information campaign programs about the benefits of PONED.
